Registration Open To All Families In The Twin Cities November 1 – December 1, 2015
Must Apply At One Of The S.N. Frazier District Churches
Emmanuel Tabernacle, Free Spirit COGIC, Great is Thy Faithfulness, True Foundation,
New Creations Ministries, Victory in Praise TWC COGIC
A child may only be registered through one charity organization.
Multiple registration by different family members, at different locations is not allowed.
You must be able to pick-up toys at the location you applied.
To be served, the following must be provided:
- Proof of Address
- Picture ID for the adult picking up toys
- ID for each child ( 0-14 years only) in the household (example: medical card, birth certificate, school ID)
*Distribution Date Will Be Provided At The Time Of Registration.
